The present study examined the effect of CPB on the release of
TNF-a as a pro- inflammatory cytokine and 1L-10 as an anti- .
inflammatory cytokine and defined the effect of pretreatment with
dexametha' sone on the release of both cytoldnes. The study included 20
patients undergoing valve replacement surgery, including 10 control
patients and 10 patients who received dexamethasone 1 mg. Kg-I 4 hours
before surgery and 0.5 mg. kg-I 6 hours later. Cytolcines were measured by
ELISA technique before treatment, one hour after aortic clamp
application, on hour after declamping and 24 hours postoperatively. In the
control group TNF- a level increased in all samples and reached its
maximal level one hour after reperfusion. TNF -a also rose in the
dexamethasone group but the elevation was less than the control group IL-
10 levels rose moderately in the control group especially one hour after
aortic declamping. Moreover administration of dexanethasone increased
IL-10 levels more than the control group. In Conclusion The present study
indiactes that CPB is a ccompained by release of both pro- and antiinflammatory
cytokines (TNF-a AND IL-10) and corticosteraids enhance
the release of IL-10.furthermor stimulation of anti- inflammatory response
is accompained by improvement of P(A-a)02. |
