The incidence of streptococcal throat infection and its complications
as rheumatic fever and rheumatic heart disease are usually
high in Egypt especially in young ages.
This study was done on 50 patients from Benha University
Hospital. Their ages ranged between 7-14 years. They were subjected
to full clinical examination, plain X ray, ECG, Echocardiography
and throat swabs to detect penicillin tolerance and typing of
oral strept.
S. pyogenes (B. haemolytic) were revealed from 22 cases
(44%) and S. Viridans (a haemolytic) were revealed from 28 cases
(56%).
Active rheumatic fever cases (9 cases); St. pyogenes was isolated
from 6 cases (66.7%) (5 cases sensitive to penicillin and one
case tolerant to penicillin) and S. sanguis from 3 cases (33.3%) (2
cases were T. and one case resistant).
Rheumatic heart disease cases (11); St. pyogenes was isolated
from 3 cases (27.3%) ( one was sensitive and 2 were T. and St. sanguis
from 7 cases (63.6%) (2 were T and 5 were R.) St. mutans from
one case (9.1%) which was T.
Cases of Tonsillitis (10). St. Sanguis was isolated from 2 cases
(20%) & St. mutans from one case (10%) and S. salivarius from 7
cases (70%) all were sensitive to penicillin.
Cases of Tonsillitis and sore throat (13); we isolated St. pyogenes
from all cases all were sensitive.
Recurrent Tonsillitis and V.S.D. (7 cases); we isolated St. Sanguis
from 6 cases (85.7%) and St. Salivarius from one case
(14.3%). All were tolerant.
