Abstract:

Ropivacaine 1% has been used in clinical Trials for peribul bar anaesthesia. The present study aims to compare the efficacy and intraocular pressure (lOP) effect of ropivcaine 1% with that of a 1:1 mixture of bupivacaine 0.5% and lidocaine 2%. Sixty patients undergoing peribulbar anaesthesia for elective cataract surgery was randomly nllacated into two groups. Group I received local injection of a mixture of Bupivacaine-lidocairte and group II received ropivacaine 1%. Hyaluronidase 15 i.u./ml was added in both groups. Evaluation was performed for onset, pain on injection, ocular and eyelid alcinesia, need for top-up injections, intra operative analgesia and 10P effect. Lower incidence of pain on injection was found in group II (P<0.05). No difference between the groups regarding the onset time. Perioperative analgesia was satisfactory in both groups but the number of patients with no pain was significantly higher in group II (P<0.05) in the first 24 hrs.. Patients in group II showed a reduced need for top-up injection and a better ocular alcinesia at 8 min. (P<0.05). The aldnesia of the eyelid was comparable in the 2 groups and complete in all cases at 8 min. 10P were significantly lower in group H (P<0.05). Peribul bar anaesthesia with ropivacaine provided better ocular akinesia at 8 min, reduced the need for top-up injections, cause less pain on injection and reduced 10P on comparing with bupivacairte-lidocairte mixture
