Aim: to compare the effectiveness and safety of LLLT, mesotherapy using homeopathic agents and mesotherapy using PRP in reversal of hair loss and stimulation of hair growth in cases of alopecia due to different causes.
Patients and Methods: The study included 60 female patients with diffuse hair falling in 3 equal groups, GpI was treated with LLLT, GpII was treated with mesotherapy and GpIII was treated by PRP, for 3 months and followed up over another 3-months- period using global photography, trichoscopy and scalp biopsy (stained for both routine H&E sections and immunohistochemistry using monoclonal anti-VEGF antibodies).
Results: GpI and GpIII showed significant clinical improvement in the global photography and HPT results (from 12.4 ±1.1 to 5.05±2.09 hairs) and (from 11.6±1.3 to 5.6±2.6 hairs) respectively, as well as total (from 123±27.2 to127.5±27.6 hairs/cm2) and (from 159.3±30.6 to 162.3±33.2 hair/ cm 2) respectively and terminal hair counts (from 101 ±29.2 to 105.5±28.9 hairs) and (from 138±34.6 to 139.6±40 hair/ cm 2) respectively, while no similar improvement was observed GpII. Histopathological examination of pre- and post- treatment scalp biopsies revealed significant increase in number of follicular units and epidermal thickness in GpI and GpIII, while patients in GpII failed to achieve comparable results. No major complications were reported.
Conclusion: Both LLLT and PRP achieved clinical improvement in cases of hair falling due to different causes. This improvement was objectively assessed and was emphasized by the histopathological interpretation of scalp biopsies. They were totally safe with minimal temporary tolerable side effects and no reported major complications, while mesotherapy failed to achieve comparable results.
