Despite impressive advances in diagnosis and management over the last decades, Heart failure (HF) is one of the most important causes of morbidity and mortality in the industrialized and developed world. It is the leading cause of hospital admissions in the adult population .
This study was conducted at Benha University Hospital and was done as a part of Euro heart failure registry which is a long term survey of acute heart failure in Europe and the Mediterranean basin. The current study included 100 patients with acute heart failure from attendants of cardiology department. patients with age < 18years were excluded from the study. The patients were observed for initial presentation, in-hospital outcome and management.
Patients admitted with acute heart failure were either acute de novo H.F (23%) or acute decompensated H.F (77%). Most of them presented with acute decompensated heart failure (59%) and pulmonary edema (30%) & less frequently presented with cardiogenic shock (7%). Most common etiologies were ischemic heart disease (57%), dilated cardiomyopathy (22%) and valvular heart disease (16%).
Overall hospital mortality rate was 6% . Forty seven patients (47%) received I.V nitrate , All patients (100%) received I.V diuretic therapy, 19% received inotropes , 3% received vasopressors, 6% received thrombolytic therapy , 52% received aspirin , 20% received clopidogrel , 57% received parenteral anticoagulant ,81% received ACEI , 2% received ARBS, 60% received BB, 68% received aldosterone antagonist, 33% received digitalis , 52% received statin , 19% received oral anticoagulant, 7% received amiodarone , 41% received nitrates and 6% received CCB.
