Abstract:

A series of 24 patients with non united fracture neck femur had been treated by a Pauwels valgu_s osteotomy in the period from 1991 to 1995. Their age ranged from 17 to 62 years (mean 46.1 years). There were 13 males and 11 females. All the patients were followed up for a period of 1-4 years ) average 2 years) after bony consolidation. The hips were evaluat-ed clinically using a modification of the Harris hip score system and radi-ologicaliy for the rate of union, the occurrence of avascular necrosis of the femoral head (AVM. Other post operative complications were also report-ed. We got excellent and good results in 14 cases (59.5%). The results considered to be fair in 3 cases (12.5%). The overall satisfactory results were 17 cases (71%). The poor or unsatisfactory results were 7 cases (29%). As regard the complications we had one case deep venous throm-bosis (4.2%) one case implant failure (4.296), two cases non union (8.3%) and 4 cases developed severe AVIV and collapse of the femoral head (16.6%). The overall rate of union was 87.5%. We think that Pauwels oste-otomy is very reliable operation in young and middle aged patients. It can give satisfactory results in many patients provided the femoral head is vi-able or there is only minimal AVN with no collapse. Introduction Non union of fractures of the femoral neck can be treated by valgus osteotomy, internal fixation combined with muscle pedicle bone graft, prosthetic replacement whether hemi or total arthro-plasty.
