Abstract:

Background:- Acute kidney injury (AKI) is a common and serious complication in critically ill patients with high mortality rate. Aim :- The aim of this study is to investigate the prognostic value of RIFLE criteria in prediction of outcome of intensive care unit patients with Acute renal failure Conclusion :- The Prognosis of patients is important in risk stratification and for efficient use of hospital resources. Predicting the outcome of patients in the intensive care environment is of particular significance, to ensure that resources are used appropriately. There are several methods have been evaluated for predicting mortality in critically ill patients . - Thus, the Acute Dialysis Quality Initiative (ADQI) group has put forward a working definition of acute renal failure. First, they had to consider the following features that would be important in any definition of ARF 1. Changes in renal function from baseline. 2. Inclusion of a classification for acute on chronic kidney disease. 3. Ease of use and clinical applicability across different centers. 4. Sensitivity and specificity relevant to different populations and research questions. - Classification system therefore should include and separate mild (or early) and severe (or late) cases. Accordingly, ADQI advocated a multilevel classification system in which a wide range of disease spectra can be included. The resulting classification, based on these considerations, is called RIFLE acronym indicating Risk for renal dysfunction, Injury to the kidney, Failure of kidney function, also includes 2 clinical outcomes loss and ESRD - RIFLE classification is as dependable as other scoring system like SOFA, APACHE II, and MODS regarding mortality prediction in patients with sepsis and SIRS presented with acute renal shutdown . - Combination of different scoring systems may be better than using a single score in mortality prediction in ICU patients.
