Introduction: Spondylolisthesis refers to the forward displacement of one vertebra relative to another. Objectives: to study the motion characteristics of the lumbar vertebral segment with lumbar degenerative spondylolithesis during weight-bearing activities in elderly patients (over 55 years old). Methods: 30 patients their age more than 55 years with lumbar degenerative spondylolithesis are included.
Vertebral kinematics are obtained using plain X ray , CT scan, MRI and dual fluoroscopic imaging technique. During functional postures (supine, standing upright, flexion, and extension), disc heights, vertebral motion patterns and instability are studied .
The cases are randomly chosen without privilege to sex , or weight, but all suffer from midline low back pain that has been proven to be attributed to lumbosacral spondylolithesis.
Results : In our study, adequate neural decompression without fixation in cases of group A with no evident range of motion shows good outcome as compared with cases of group B treated with decompression with instrumentation using transpedicular fixation with poster lateral fusion.
Conclusion: Lumbar DS is a degenerative disease of lumbar spine results in neural compression but does not result necessarily in vertebral instability, so some cases of lumbar DS may need only neural decompression as restabilization process may have occurred but other cases show instability which requires decompression with instrumentation using transpedicular fixation with poster lateral fusion
