Abstract:

Background: The syndesmosis is a fibrous joint linking two adjacent bones by strong fibrous ligament or membrane. Inferior tibiofibular syndesmotic injuries result from external rotation with dorsiflexion injuries. Displaced injuries and injuries associated with fractures are managed operatively by reduction and stabilization of the syndesmosis, recently tightrope was used as alternative fixation modality with less side effects. Aim of the Work: Evaluation of clinical and radiological results of Tightrope fixation of syndesmotic ankle injuries without fractures. Patients and methods: prospective study of 21 cases of isolated syndesmotic ankle injuries done in Benha University Hospital and Health Insurance Hospital between May 2012, and November 2015. Patients were evaluated clinically to document the mechanism of injury, inclusion criteria and exclusion criteria then confirm diagnosis using imaging techniques ending by surgical repair using Tightrope fixation modality. Results: Satisfactory outcome was achieved in all the patients with average postoperative AOFAS hindfoot scoring of 51.5 (range, 31to 62), with all our patients returned to work in 4 months postoperative with no need to second stage to remove hardwere . Conclusion|:Tightrope is an effective technique in management of isolated inferior tibiofibular syndesmotic injuries with favorable outcome and low complication rates.in addition to one stage procedure with no need to remove hardwere.
