Abstract:

Aim: prospective study comparing SIMV; PSV and SBT methods of weaning from mechanical ventilation in head injury mechanically ventilated patients Methods: 120 head trauma mechanically ventilated adult patients. They were classified into three groups each included 40 patients. Group I patients were weaned using SIMV method ,Group II were weaned using PSV and Group III patients were weaned using the SBT after passing the spontaneous breathing screening .The primary outcome of the study was extubation for not less than 48 hours duration without re-intubation. Results: No significant difference between the 3 groups regarding the demographic data. Insignificant difference was found between the 3 groups regarding the 3 main secondary outcomes which are the ventilation duration, ICU length of stay, and the hospital length of stay, all in days. Although the rate of reintubation which is the primary outcome showed insignificant difference between the 3 groups, but with using Fisher’s Least Significant Difference Test, we found that there was significant difference between group I and III and between group II and III with no significant difference between groups I and II with the least number of reintubated patients within 48 hours were found in group III. Conclusion: inspite of there was a little difference between the compared 3 methods of weaning of head injury patients from mechanical ventilation in this study, but we recommend the adherence to a well-organized protocol of weaning represented in the SBT method.
