Abstract:

Objective : To evaluate reproductive outcome after hysteroscopic metroplasty using simple mechanical instruments and electroresectoscope. Design : The cumulative proportions of women who became pregnant and gave birth, subdivided according to the type of septum and the primary indication are compared regarding the technique used for metroplasty. Setting : University based and private practice tertiary referral practice. Patients : Forty six consecutive patients with fundai septum (n = 29), cervico-fundal (n = 14) and vagino-cervico-fundal septum (n = 3) referred because of infertility (n = 10) or repeated abortion (n = 36). Investigation : Hysteroscopic raetroplasty performed by scissors (n = 25), electroresectoscope (n = 16) and cold knife (n = 5). Main Outcome Measures: Postoperative cumulative pregnancy rate. Results : The pregnancy rates were 72% and 56.2%, respectively after the use of scissors and electroresectos-core. Conclusion : Although metroplasty using the electroresectoscope is technically easier yet this small series suggests that the pregnancy may be delayed in aooition cases and may be less achieved in infertility patients after metroplasty using resectoscope than scissors. The use of cold knife with selective coagulation of the bleeding points is suggested to decrease the thermal effects on the endometrium during hysteroscopic metroplasty.
