Bottle neck press fit of hamstring tendon could be a reliable method of femoral fixation in arthroscopic anterior cruciate ligament reconstruction. Patients and methods: A prospective study was conducted on 28 patients with symptomatic ACL tear treated by press fit hamstring ACL Reconstruction. After harvesting the hamstring tendons, the free graft was knotted on itself at its middle. Femoral tunnel was created in inverted bottle neck fashion. The graft was fixed to the femur by press fit fixation and fixed to the tibia by biodegradable interference screws. Patients were followed for a minimum of 5 years. Results: Satisfactory results were obtained in 92.9 % of patients who scored normal (A, 18 patients) or near normal (B, 8 patients). All the 20 athletic patients included in the study returned to the same type of pre-injury sport activities, 16 of them to the same level (normal), and 4 to returned to a lower level (near normal). Eight patients returned to their pre-injury occupations as they did not share in sport activity. Two patients had dissatisfactory results either abnormal (C, one patient) or severely abnormal (D one patient). Conclusion: Press-fit bottle neck fixation of hamstring tendons in femoral tunnel offers satisfactory stability with a reliable outcome and avoid possible abrasion or fraying of the graft. |
