Abstract:

The correlation betweenDNA cell Image Analysis, AgNOR count and PCNA LI in hydropic products of conception. Clinical Impacts. Hala A. Agina, Ph.D.; Ragaey R. Fahmy MD ; Samia A. Afify, M.D. Departments of pathology, and Gynaecology & Obestetric, Benha faculty of medicine Benha University a b s t r a c t The significance of cell image cytometry (ICM) { DNA index (DI), histogram, proliferative activity determined by percent of cells in S-phase (PA), cellular morphometric measurement of villous and nuclear areas} argyrophilic nucleolar organizor region (AgNOR) count and proliferating cell nuclear antigen immunoreactivity (PCNA) in the classification and evaluation of hydropic placetae were studied. Serial sections of 44 cases from paraffin embedded tissue histologically classified as 11 partial mole (PM), 18 complete mole (CM), 9 choriocarcinoma and 6 cases of spontaneous hydropic abortion were examined. DNA index (DI) and PA determined by ICM showed a high statistical significant difference between hydropic abortion, PM, CM, and choriocarcinoma groups (P> 0.001). a case of hydropic abortus with diploid histogram showed high PA (> 32%) was revised to molar pregnancy. Also, morphometric measurement revealed significant difference in mean villus area (MVA) and in the mean nuclear area (MVA) between studied groups. By using both mean AgNOR counts an PCNA labeling index (LI), analysis of variance showed a high significanct difference between hydropic abortion, PM, CM, and choriocarcinoma groups ( P< 0.001). PCNA LI was more sensitive than AgNOR counts (F ratio 190.3 versus 9.8). AgNOR counts showed a significant positive correlation with ploidy (r=0.91), DI(r=0.85) and PA(r=0.61043). also, PCNA LI showed significant positive correlation with ploidy (r=o.75), DI(r=0.73) and PA (r=0.67). a significant positive correlation was found between AgNOR count and PCNA LI (r=0.58). This study showed that ICM, AgNOR count and PCNA LI are valid methods of determining ploidy and proliferative activity of hydropic placentae to classify them and could be of value to predict their prognosis. PCNA LiI is easier than ICM and more sensitive tha AgNOR count differentiating hydropic products of conception and in predicting their course. Research in C M B. 6 (4)(1999): 1359–1373
