Respiratory distress syndrome is one of the most common causes ofrespiratory distress in newborn. In preterm infants with respiratory dis tress syndrome (RDS),cardiac function is negatively influenced by .he severity of the lungdisease, cardiac troponin T (cTnT), which s an inhibitory proteincomplex located on the actin filament in all striated muscle and consistof three subunits T,e, and I. is a biochemical marker of myocardialinjury, its high concentration in preterm infan:s with RDS suggest thepresence of myocardial damage in this group pa tients. In this study we used cardiac troponin T as a biochemical marker todetect myocardial injury in infant with RDS. This study included 50preterm neonates (28 male and 22 female). Their gestational age rangedfrom (28 weeks to 36 weeks) and their weight .anged between (1500gmto 2900 gm) they were divided into two groupsThe I” group: will include 10 cases of healthy preterm neonates who donot have respiratory distress. The 2nd group: will include 40 cases of preterm neonates withrespiratory distress syndrome. Were we found there was no statistic 11 significant correlationbetween troponin T level (TnT) level and sex distribution, P. value0.870.There was no statistical significance correlation between TnT leveland gestational age, P. value 0.098.There was no statistical correlation between TnT level and the bodyweight, P. value 0.129.- 81-SummaryThere was no statistical significant correll tion between TnT leveland maternal age, P. value 0.911.There was no statistical significant correlution between TnT leveland mode of delivery with P. value 0.535.There was no statistical significant corre ation between the leveltroponin T and CBC (P. value are O. 157 for Fb level, 0.937 for RBCscount, 0.404 for leucocyte count and O.512 for platelet count). There was high statistical significance neg rtive correlation betweenTnT level and with arterial blood PH with 1’. value 0.001 and witharterial blood O2 with p. value 0.013 and witt arterial blood C02 withP.value 0.001 and with arterial blood HC03 w th P. value 0.001 amonggroup2.There was high statistical significance increased of TnT level(ng/rnl) among RDS infants, group 2 infants with P.value < 0.001 andthis indicate that there is myocardial affectio 1 among group 2 due toRDS. This results may show that TnT is a good marker for myocardialischemia and as marker for diagnosis ofRDS ir neonates. Also we found a significance difference iJl cardiac troponin T levelbetween infants with and without respiratory di stress. |
