It was tried in this work to understand more the pathogenesis of diseases in the maxillary sinus. It was discussed briefly the anatomy and physiology of maxillary sinus then we introduced different diseases of the sinus.57 patients were selected with different maxillary sinus insults, the different diseases were the commonestaffecting the maxillary sinus.A special instrument was invented to measure the pressure inside the maxillary sinus, this instrument was tried and tested by specialised persons to proove the effectiveness and ability of this instrument for its purpose.Measurement of pressure was performed in 94 maxillary sinuses because 37 patients had bilateral and 20 others had unilateral pressure measurement.The results were discussed in view of clinical and radiological diagnosis. It was found that, pressure in the maxillary sinus is active - positive or negative - when the ostium is closed and pressure is zero - atmospheric -when the ostium is open.
