This work represents the study of 30 cases of senile enlargementof prostate.The work comprises an analysis of the radiological findingsobserved in the upper and lower urinary tract in cases of senileprostste and a study of the changes following prostatic surgery includingreversibility of the preoperative changes and the chan~es inthe bladder neck re~ion with correlation to the clinical findingsof the function Q., recovery or complications.Our results points to the following:1. The filling defects produced by the obstructing prostate werefound in 14 caS2S out of 25,the defect is rnever notched inoutline.2. Generalised elevation of the inferior margin of the bladder wasseen in 2 of the cases.3. Trabeculations of the bladder wall were reported in 15 cases outof 22 adequately visualized bladders. small diverticula wereseen in 2 cases and muscle hypertrophy ·in 7 caaes. These vesicalchanges were crudely parallel to the severity of the symptoms.4. Vesco ureteric reflux was sean in one case out of 10 cases examinedby the ascendinG cystography.5. Changes in the upper urinary tract were found in II cases outof 25.6. Post operLtive prostatic cavity was seen in all oases. Muchdiminution of the 08vity was deteoted in films taken 6 monthsafter the oper,tion. Complete inclusion of the cavity ooouredin one case due to stricture membranous urethra whioh oomplicatedthe operation. Reflux up the ejaculatory ducts was demonstrated7. lIfterthe cper ati.on the trabeoulations were either greatly attenuatedor lost. The diverticula dis not show any ohange of thesize of the pouches. Vesico urteric reflux which was demonstratedin one disappeared after prostactomYr8. In 3 oases the dilatation of the upper urincry traot was followedpostoperatively and it persisted in 2 oases out of the 3.9. In the 10 oases followed pre and post operLt ively radiologioalevidenoe of oomplioations was not present. Out of the 15 oaseswhose reoords were present oomplioations were found in 3 oasesmonths after the operation.10. The follow up of the oaSLS helps together with the olinioalfindings in the assBssement of the suoess of prostateotomy. |
