-97SUMMARY AID CONCLUSIONOtitis media with effusion is one of the commenestchronic otological conditions of childhood • It resultsfrom altration of the Mucociliary system with middle earcleft where serous or mucoid fluid accumUlate in associationwith negative pressure.Although there is no signs of inflammation, bacteriacan be cultured from effusion in as many as 50 % ofcases.An effusion frequently remains in the middl~ earfollowing ~~cute supparative otitis media but usuallyspontaneous cl~arance within a fe1.we~ks.The condition occurs in childhood as overtor covert Hearingloss presenting as an eductional problem.In younger childernit may present as speech or longuage delay.Fifly children were selected to have SOM after exclusionof cases with congenital or heridetory abnormatitiesor sensori- neural defness.These cases were treated by antibiatics,antihistamin_ics,nasal DROPs,mucolytic and steroid for one month- -- -- - - - - -- -- -- ---- ---- - --- ------------ 98 -followed every week during treatment and after three mon_ths for the improved cases.The improvaent rate at the end of treatment was 17case 34~ from the tot_algroup’.k 33 cases (60%) sho.wnoimprovment and resorted to surgical interference.Fourteen cases out of the 17 cases Were of acutetype of SOM and the remaining 3 cases were of chronicS 0 M •After three monthes re- examination were done forthe improved 17 cases. 5 cases showed recurrence of SOMand 12 cases showed no recurrence.from the results of this work. it can thus be cancludedthat the medical treatment is more effective in acuteSOM than chronic SOM. SO it is better that medical treatmentshould be tried in every case of SOM before surgicalinterference. |
