The morphologic changes in rectal bladder afterradical cystectomy in bilharzial cancer bladder patientsis studied for a period ranging from 6 monthes to morethan 6 years. We found that some adaptive changes weredemonstrated in the form of pronounced atrophy andloss of goblet cells and reduction of mucin in the crypt •That is the cause of decreasing mucous secretion whichpasses with urine after the rectal bladder in placepost operative by about 6 monthes. There is also chronicinflammatory changes were found. Marked flattening ofrectal mucosa and replacement by adaptive metaplastictransitional epithelium. The importance of such adaptivechanges is the protection from urine absorption andlowering the icidence of biochemical disturabances andacidosis. Some hypertrophic changes were found by authorsin the form of benign hypertrophy in the form of a polypand also malignant changes in the form of carcinoma,(adeno carcinoid tumor goblet cell type). Uncontrolablebleeding may be serious presenting symptom of thesepatients. Follow up endoscopy is by far the most importantpoint in the follow up post operatively best done every6 months to detect early serious malignant changes beforecomplications establishement. |
